Comprehensive Diabetes Mellitus Care Integrated with Primary

Sivam Diabetes & Primary Care, LLC is a health care clinic focus diabetes mellitus care. The health care provider is Dr. Mavis Ermakov, DNP, APN, FNP-BC, RC, CDCES an experienced and well-respected  Nurse Practitioner (NP).  Dr. Andrew Baddoo, MD, an Internal medicine  and nephrologist is  her collaborating physician.

Sivam Diabetes & Primary Care (SDPC) LLC. envisions the provision of evidence-based, culturally sensitive, high quality primary care with a focus on diabetes care (Diabetes Self-Management Education and Support (DSME &S), including medical nutrition therapy (MMT) to the target population:

  • Lead innovations in developing health, education, and diabetes mellitus.
  • Implement interventions with the roles of nurse practitioners/healthcare providers to meet the changing needs of the area’s target population.
  • Catalyst in collaborating with other organizations to develop a comprehensive network of services for the target population in our community and integrate behavioral health care to address the mental health needs of the target population.
  • Secure adequate funding to serve the growing and diverse target population.
  • Provide leadership in promoting the well-being of the target population.
  • Empowerment of clients through high-quality healthcare interventions.
  • To improve the health of individuals, with a focus on diabetes mellitus care and hence improve the quality of life for people with diabetes mellitus.
