Dr. Mavis Ermakov’s background, experience from years of practice, collaborations with other specialist physicians, and training as a multi-disciplinary healthcare provider makes her uniquely qualified to care for patients.
Dr. Ermakov is an advocate for health and wellness, keen on promoting health literacy and access to healthcare for all. She has been trained as a Registered Dietitian, with multidisciplinary training as a Board-Certified Family Nurse Practitioner, and Certified Diabetes Care and Education Specialist (CDCES). In conjunction with more than thirty (30) years of experience in healthcare in various settings, she is a primary care provider and has essential diabetes skills to adapt care and nutrition intervention that is culturally sensitive to meet a wide variety of health needs from infancy to senior care in the primary care setting and long-term care settings.
Dr. Ermakov has a passion for taking care of patients with diabetes mellitus. Diabetes Mellitus is at an epidemic level in the United States and is the major cause of many acute and chronic complications. Specialty areas are Diabetes Mellitus; Primary Care, Obesity; Hypertension; Cardiovascular disorders (High Cholesterol), Kidney Disease, Gastrointestinal disorders; before and post Bariatric surgery, Child Nutrition, Geriatrics and overall Medical Nutrition Therapy to promote healthy eating.
Her focus is to help patients to access healthcare with a focus on diabetes mellitus care to promote healthy living for improved life performance from infancy to adulthood. The office/clinic is handicap accessible with adequate parking and our location provides easy access to public transportation.
We treat patients who have Type 1 Diabetes Mellitus and Type 2 Diabetes Mellitus, Pre-diabetes, secondary diabetes mellitus, and diabetes mellitus during pregnancy (gestational diabetes mellitus).

Services such as medications refills and treatment of simple conditions such as UTI, cough, allergies, minor skin rashes/conditions can be provided with the use of telehealth.

NEVER REQUEST AN ELECTRONIC VISIT FOR A EMERGENCY MEDICAL PROBLEM.
An electronic or e-visit is an alternative designed to efficiently respond to routine, non-complex medical problems. (Examples might include mild
cough, a mild stomach virus, UTI).
An e-visit is not designed for complex or non-routine medical care, especially problems that might require the relating of extensive history information or a thorough physical exam. E-visits are only offered to ADULT patients, and you agree that during the visit you are representing yourself and not another person. You will be asked to present a government issued ID. E-visits are not billed by our practice to any insurer, as they are usually considered a non-covered service. The patient is always responsible for our typical e-visit charges. Requests for e-visits must be approved and confirmed at least 1-2 hours in advance. Prior to the visit you may be asked to complete certain medical questionnaires. Sometimes, after reviewing your information, or during the e-visit it may be determined that your problem is too complex for an e-visit session. In that case, our office will schedule you for a traditional office visit and your e-visit fee will be applied to the patient balance related to your office visit.
Communication during an e-visit may be exchanged via teleconference, landline phone, cellular phone, and online chat. These methods are by their very nature not as secure as a face-to-face encounter. By requesting an e-visit you acknowledge that some personal health information will be communicated in a manner that is not encrypted. Please refrained from emailing personal information aside from your age, gender, and basic reason for your visit. You will be granted access to the secure portal as part of the registration for more detailed information.
As with any medical service, decision, or treatment, there are risks; and an e-visit is no different. Because this visit is electronic and without physical exam, you acknowledge that the risk may be greater than a traditional office visit. You agree to schedule/seek follow up as advised by the provider and abide by all home care instructions given during the consultation. By requesting the visit, you agree to accept the outcome-even if it is undesirable and you will not be entitled to a refund for services.

A Registered Dietitian provides all nutrition care. Individual nutrition counseling and follow-up nutrition care is provided on an outpatient basis for disease-related nutrition problems, including, but not limited to:
Individual nutrition intervention (MNT), allows the provider and patient/client the opportunity to plan a tailored nutrition program that will allow nutrition and long-term lifestyle changes to meet health goals. Daytime and evening hours are available for appointments to meet your needs.
Why should I see a Registered Dietitian (RD)? Can I have sugar because of a diagnosis with diabetes mellitus or what can I eat? These are often the first thoughts that you may have when you are diagnosed with diabetes, celiac disease or are struggling to manage your weight.
A Registered Dietitian is the nutrition expert who can help you choose practical solutions for healthy eating based on your individual needs. Your dietitian will work with you to develop a meal plan, read labels, or figure out how to eat out successfully.
Registered Dietitians at our clinic can help you with diabetes, weight management, cardiac disease, GI disease, eating disorders and many other nutrition-related conditions. Talk to your doctor about a referral to see a Registered Dietitian and check with your insurance company to determine if nutrition counseling is a covered benefit. Diabetes Self-Management Education and Support (DSMES). DSMES is “the ongoing process of facilitating the knowledge, skills, and ability necessary for diabetes self-care, as well as activities that assist a person in implementing and sustaining the behaviors needed to manage his or her condition on an ongoing basis, beyond or outside of formal self-management training.”
